Multi Agency Collaboration

Multi Agency Collaboration
Working Together to Meet Every Child’s Needs
At the Meadows School, we believe that the best outcomes for pupils are achieved when professionals work together around the child. Our approach is built on strong multi-agency collaboration, ensuring that every pupil’s educational, health, and care needs are met in a coordinated and holistic way. By combining educational expertise with therapeutic input and safeguarding oversight, we create a safe, nurturing environment where pupils can communicate effectively, build independence, and thrive both in school and beyond.
A Team Around Every Child
Each pupil’s learning journey is supported by a team of dedicated staff and specialist professionals who work closely with families. This includes:
- Speech and Language Therapists (SALT) – supporting communication, interaction, and social development.
- Occupational Therapists (OT) – helping pupils develop fine and gross motor skills, independence, and sensory regulation.
- Physiotherapists – promoting mobility, posture, and physical wellbeing.
- Complex Care Nurses (CCNs) – ensuring that medical and health needs are fully supported within the school environment.
- Educational Psychologists, Social Workers, CAMHS – contributing expertise to assessment, planning, and intervention.
- Careers Advisor (Connexions Service)- providing impartial advice and guidance to support transition and post-16 pathways that reflect pupils’ strengths, interests and aspirations.
- Central Support Services- offering specialist input for learners with hearing impairments (HI), and visual impairments (VI) ensuring access to learning and inclusive classroom practice.
- Pastoral Team- providing emotional support, safeguarding guidance, and wellbeing interventions to help pupils feel safe, valued and ready to learn.
- External Safeguarding Consultant – offering independent advice and training to strengthen safeguarding practice and ensure the highest standards of pupil protection and wellbeing. The consultant recently delivered PACE (Playfulness, Acceptance, Curiosity, Empathy) training to staff, supporting trauma-informed approaches and positive relationships with pupils.
Collaborative Planning and Support
Multi-agency working is embedded into every stage of a pupil’s education, as appropriate to their needs. Joint meetings, classroom observations, and shared target-setting ensure that advice from professionals is translated into daily classroom practice. Personal Learning Goals and outcomes are reviewed collaboratively, allowing everyone involved to contribute to each child’s progress.
Teachers, therapists, and families communicate regularly to ensure continuity between home and school.
Putting the Child at the Centre
Every decision we make is guided by what is best for the child. We value pupil voice and family input as essential parts of the planning process. By working together, we help pupils develop the confidence, communication, and life skills they need to thrive both in school and beyond.
Our collaborative, child-centred approach ensures that every learner has the support and opportunity to reach their full potential — academically, socially, and personally.
